Most private weddings need a clear notice, not signatures: a line on the website and a sign by the QR code telling guests what is collected and where it goes. A signed consent form is for the cases beyond that: photos used by the venue, planner or photographer for marketing, photos of children, or a guest who wants their agreement in writing. Notice or form? Choose by how the photos will be used
A wedding gallery seen only by the couple and their guests is a private activity. Guests expect to be photographed and expect the photos to be shared among the people who were there. What they need is a notice: what is being collected, who can see it, whether there is a live wall, and who to ask for a photo to come down. The consent wording guide linked below has that wording for invitations, signs and the gallery itself.
A signed form matters when a use goes beyond the guest list. The venue wants reception photos for its website, the planner for their portfolio, the photographer for advertising, or the couple for a public account with a large following. Those are the cases where a person can reasonably object, so ask them properly and keep the answer.
What the template covers
Event details and a contact for removal requests, so the form works on its own. A short list of what is collected, including guest uploads and the names shown in the gallery. Tick-box uses from 'private gallery for guests' to 'advertising by a named vendor', so a guest agrees to specific uses rather than everything. A withdrawal clause that is honest about what cannot be undone. A children's clause with a parent or guardian signature. A declaration and signature lines.
Anything you do not intend, delete. A form that lists marketing uses you will never make only raises questions.
How to use it without making the wedding feel like paperwork
For most guests, put the notice on the website and beside the QR code and skip the form. Send the form only to the people whose photos will be used beyond the guest list, usually after the wedding when you know which photos those are. For a venue or vendor request, ask the vendor to handle their own releases; it is their marketing.
For children, a parent's signature on one family form is enough. Keep signed forms with the rest of the wedding records, and delete them when the last permitted use is over.
Jurisdiction notes
In the UK and the European Union, consent for using a person's image has to be specific, informed and withdrawable, and photos of private events are personal data with the usual rights attached. The GDPR answer linked below covers what that means for hosts. In the United States, commercial use of a recognisable person generally needs a model release, and rules vary by state.
This template is a starting point drafted for private events. If a photo will be used in advertising, press or anything with money attached, have the release checked by someone qualified in the relevant country.

Do wedding guests need to sign a photo consent form?

Can the venue use our wedding photos on its website?
